So, at least for me, the problem only comes back when I add friends to my "favorites" list. Right when I do that, the artwork freezes again, the shuffle and repeat buttons stop highlighting etc. I remove my favorites, log out and log back in and it's back to normal.
The problem has to be more than that if some people still can't fix it with that trick and there are some that don't even have a friends list.
This is pretty bogus.

Let me add to the list that when the album art is at it's larger size (you can minimize it or make it larger) I can't scroll all the way down my playlists. The artwork blocks the latter part of my playlists.

I got a response back from support that suggested clearing out my preference files, deleting the app and installing the preview version. That didn't work, so I ditched the preference files again and went back and downloaded the regular version.
And things worked... for about 5 minutes.
The album artwork was changing. The track names were changing. The play/pause button was working. I was getting Growl notifications. Cool.
But none of the apps worked. I could only launch songs from my playlists. I couldn't go to 'Rolling Stone' and launch a song for example.
So I restart. Now back to the same old thing. Album art doesn't change. Play/Pause button doesn't work correctly. No Growl.
Grumble.
I promise to pay, Spotify... seriously I will. If you'll promise to release a version that actually works as it's supposed to.

You know what REALLY gets me about this? Not only are they virtually ignoring this issue, when asked for some sort of good faith compensation, I received this reply:
While I cannot issue you a refund at this time because you've been logging into and using the application, what I have done is stopped future payments on your account.
You will continue to receive our Premium service until 11 April, but then you card will not be charged again. Of course if you'd like to continue with Premium after that date, all you need to do is go to the subscription page and upgrade again: https://www.spotify.com/account/subscription/
Which translates to me: **bleep** you, we don't need your money anyway. Bad business practices, guys.

I am having the same problems (radio doesn't advance at all, "now playing" doesn't ever advance past first song), and I am beyond fed up. We are paying for this subscription, and to be completely ignored by Spotify is horrible horrible customer service, a complete abuse of the people who are making your business possible. I have wasted so much time uninstalling, reinstalling, and looking helplessly for any fix on line. We already know that Spotify is paying the musicians/labels hardly anything. Well they're clearly putting nothing into testing, development, or customer assistance. So where is all the money going?I've invested so much time in this it's difficult for me to cancel, but I certainly will not be upgrading until this is fixed and we start to get better customer service. And I will JUMP at the first competitor who gives us any reasonable alternative. Thanks Spotify.

Came here to say something similar:
Even with the temporary semi-fix it still comes back every time you relaunch spotify, which means (at least for me) I have to logout and log back in to get the artwork to update and the buttons to highlight.
I did have the radio app working perfectly earlier. Now I can't even get the radio to play a song at all. It says "Track Not Found".
So, who knows of a good alternative to Spotify. I've heard Grooveshark has a paid service but I've never used the free one at all. Any suggestions? I will gladly throw my money at any other service that can provide me with full albums.

grooveshark free is great, minimal ads, you can select a song to play (unlike pandora or last fm which plays ranges of songs) and play radio.haven't upgraded yet to paid. still waiting to find that absolutely perfect service (ha!). grooveshark is pretty close to spotify, but a bit clunky. and web-based.
pandora and last fm are great for radio based on an artist/genre.
