Are you absolutely certain that this is happening in every case ?
If so, it is a bug that will need to be investigated. However, remember that any recording will appear twice in your library if it is sourced from different albums (ie different URIs). Are you sure that the track you have added to a playlist is from the same album as the one you have starred ?
Post the URIs of both appearances in your library list here.
I've just figured it out, It's all the songs I've "starred" from my the last.fm app that i've "loved" The album names are all the same but they're different versions, i.e the two versions of "the suburbs" by Arcade Fire that are on spotify
