I have two single Spotify Premium accounts in my house. The promo worked fine for my daughter. Mine will only populate a US address option for a Canadian account. The reply below that provides a link to a very long thread of people complaining about family plans didn't help. How do I get the Google checkout to switch to a Canadian address?

I finally figured out this issue with a little guidance from the support team at the Google Store. Check your setting in your "Payment Profile". I had never set it up so it was defaulted to the US.

To confirm, did you get this issue resolved? If you're still needing help, please make sure that you have accessed the Canadian version of the page. It should have /ca-en/ in the link and not /us/.
