My son has free and wants premium. I see that you can use a gift card to upgrade from free to premium, but does it matter if the gift card is a "regular spotify" or a "premium spotify " gift card?

There is really only one kind of gift card and it's for Premium Individual. The only difference is the number of months you get from it, which you'll see written on the card itself. Full info on gift cards here.

Where do I find the monthly cost for premium? I found the 12 mo ones, but
wanted to give him a month or two to see if he likes it first.

This depends on the retailer and availability. There should be gift cards for 1, 3, 6 and 12 months, so you'll be able to get them for the short term too.
