I have a similar issue. I have my "open Spotify automatically" option set to minimize, but it opens windowed. Every time. Sometimes it opens fullscreen. My "open Spotify automatically" setting appears to have no influence on whether Spotify opens at launch.

Do you have Spotify turned off in your startup queue? (Ctrl+Alt+Del -> Startup tab)

The other place to check, if you haven't already, would be the startup folder. (Win+r -> "shell:startup" or "shell:common startup" + run) I believe it is empty by default. Useful for adding startup programs that don't pop up in the startup queue (or whatever it's called).
