Spotify won't stream via bluetooth - then won't work when disconnected

I have Spotify Premium on an HTC Desire S. When I connect it to my Samsung audio unit via Bluetooth it occasionally works, but more often it says it is connected, the song looks like it is playing, but there is no sound. If I then disconnect from Bluetooth, I get no sound at all from my phone although the song is still playing.

It works when I restart the phone but obviously this is not a good solution long term - any clues?

I see Bluetooth has been a big issue for Spotify for a long time 😞
