Re-installed android app to fix some connection issues. Now there is no way to log out or change preferences. I am logged into my premium account. I am currently on Android 6.1

What actually happens when you tap the hamburger menu bars at the top left or swipe the screen to the right? Does the settings icon (a small cog) not show at the bottom tight of the screen?

Attached is a screenshot of my home screen in Spotify. I don't see any "hamburger", and when I swipe right nothing happens. I have unistalled and re-installed from Google Play and Spotify and get the same results. I logged in with my Spotify Premium account. It allows me to download to my phone, but that is about the only advanced functionality that I get.

Hamburger bars are gone and replaced by the row of icons at the bottom offering the same functionality as before with an easier to reach search button.

Which brings me back to my original dilemma, how do I change settings?

They cunningly concealed the settings icon in the Your Library page. You'll see it there in the top right corner.
