2 accounts under 1 username

Have 2 accounts under the same username, but with different passwords. The first one was with a premium account (via KPN), and the 2nd one was created (without my knowledge) by a family member with the same username but now for temporary free premium period. This 2nd account was created for Sonos.

Can you arrange that the 2nd account is cancelled, and that my first account (premium, again via KPN, username buijkspot) gets the premium status again?
