2 users on my account

Hi,
Me and My wife has 2 different premium accounts.
Problem is we cannot be logged in at same time... Does anybody know how it comes that it is like this?
We have total different logins and only common thing is that i see her nickname when i go to settings and i see her music at My login.

My ex-boyfriend is still using my account, I've tried to change my password--- log in & out. It still doesn't work. Is there a way to reset my account or something so he won't be on it anymore?
