Account Hacked

An unknown iPhone has been using my account today. I have signed out of all devices and changed my password to prevent further logins.

 

The iPhone in question came up as "Long's iPhone".

 

Please can you advise how long this device had access to my account and where did the iPhone originate from?

My account has been hacked, I tried to get it back but it seems like the hacker has changed both the email and password. It is a premuim account so i wondered if there is a way to delete that account or unsubscribe. Accounts name is PPZEREALPP. I also saw that he made a new playlist on my account and deleted mine. So it is an active user.
