Hello, I choose 12 months and fill in the form and then click continue. Then I just come back to the same page with a empty form.

Tested with both Google and Microsoft Edge.

Are you still unable to purchase that e-card? If so, try clearing your browser's cache/cookies, or you can also use an incognito window on your current browser and see if it helps.
