When I try to log in I get the following message "Login failed". "You have already created an account with this email address"

That doesn't sound quite right. 

Have you tried to use your username instead of your email address in the login process?

This solved the issue in the most cases. ^^

I apparently have forgotten my user name for this site.
When I tried logging in through Facebook I got "Login failed, error 404.

No it didn't solve the issue. I created a new account with a different emai address instead.
