My account has been reset to a free account but I am still paying for a premium family

My Question or Issue

Last week all my playlists disappeared on my phone.  I logged in on my laptop to see what was happening, and it says that I am on a free account even though I have been subscribed to a family premium account for over a year and am still paying for it.   I'm not sure why it has suddenly changed or what I can do to get my paid account and playlists back - please help!
