Someone connected my account with their Facebook without my consent

Now my name and profile pic are theres and I can't get rid of it. I also think he's able to listen to music with my account, as sometimes my friends who are following me say they can see me listening to some weird stuff.

I've tried following instructions to unlink the FB account, but there's a step I can't carry out - in settings there should be a "disconnect from Facebook" option but there isn't, only a "connect to facebook" option. 

Anyone have any ideas on how to undo this?
