Someone is using my account from an unknown device

My Question or Issue

So basically, when ever I use Spotify it disconnects me from my music and says it’s playing from another device(web fox) and when I press forget device it always comes back. It will change my playlists and songs. Sometimes my account logs out. Is it beginning hacked? I have gone on to Spotify and it’s already playing on another device before I start my own music. No one I know, knows my password so it can’t be my family using my account.
