Unrecognized Device Using Account

Hello,

 

I'm having a problem that an unrecognized device keeps using my account. Has deleted my playlists, interrupts songs, and creates playlists. I tried doing the log out everywhere option and have changed my password twice now but as soon as I'm logged in it asks which device and it's the same unrecognized device trying to use my account. Name of unrecognized device is iPhone di Orazio.
