Maybe that's happening because your subscription via iTunes hasn't expired yet. I'd recommend waiting until it's expired and then try to subscribe to Premium for Family 🙂

Have you tried to edit your subscription through the Spotify Account Overview? This area will allow you to manage your subscriptions and payments. I think it is also cheaper when completed through the site because Apple put some form of 'service charge' to the subscription. However if this doesn't work pop them an email or direct message @SpotifyCares on Twitter so that they can help sort out the issue with you!
