help logging in with fb

I accidently removed Spotify from my fb and now it won't let me log in. I'm not 100% sure what my username is and I tried emailing support but received nothing. I want to get premium but don't want to if I cant hook up to my fb contacts. Is there anyway for me to get this information? Or a way to re add the app on fb ?
