You can cancel your subscription at your subscription page. If everything goes well you'll receive a confirmation email shortly after confirming that you've cancelled your premium subscription. 

As for the refund please get in touch with Spotify via the contact form or DM/Tweet @SpotifyCares.

Their support team will be able to take a look at your issue and you'll hear from them shortly.
