Songs skipping and random songs playing

The songs I play skip randomly and songs that I'm not interested in are interupting my choice of tracks constantly and randomly. 

Could I have been hacked into? I don't recognize most of the recent activity listed on my homepage and I can't remember the last time I heard a song of my choice in full without it being skipped.
