Lost Premium Account, Probably due to duplicate account

So I've lost my premium account and playlists for the 2nd time since I started using spotify.  I've tried the logout log back in with facebook method but that has not worked.  When I try and use my original spotify name, account was created before sept 2011, it says I have an invalid password.  Whenever I try to recover that password the website tells me to log in with facebook or change my facebook password.  I'm not sure what else to try.  Also, my mobile app has switched to a premium trial, however it still has my playlists for some reason.

Contact support and wait 3 to 5 days for a response.

 

If that fails contact your credit card issuer for a refund.
