Account connnected to unkonwn device

Since yesterday my account is connected to a PlayStation 4, but I have never connected with any PlayStation 4 . But, when I try, with my phone, to connect with that PS4, nothing happens, that device doesn't even show on offline session. I need help to know what happened or, at least, disconnect that device from my account.

So far, the only thing I have done is changing my password.

To sign out of all devices please perform the following:-

1. Go to your account page.
2. Click SIGN OUT EVERYWHERE
