When you put shuffle on, every song that has been played by shuffle afterwards in a playlist should be grey'd out (marked as already played) so they can't be played again, that way you force it to run through the entire playlist, but in a more random order.
Could be added as an optional shuffle in the settings or something for those who doesn't want it.
Even if they are greyed out, you can still play them if you click yourself or add them to the queue again, perhaps an option to "un-grey" by right clicking.
