Once 4-5 months family plan removes me automaticly

My Question or Issue

 Once 4-5 months family plan removes me automaticly, before today,my dad was inviting me again, but now it is not working. I can not join my family plan again.(We are living in same house)

It says you couldn't join this family plan.Do you need help? Read the SSS part.

I am sure that my informations are true.

I checked all these informations, but I don't know what is the problem.Why it is not working.Are there anybody who can help me?

I used approximate 1 year same family plan.Can anybody please put my account to same family plan?
