Payment betwee familiar plan and premium

If I just paid the month of Spotify Premium and I change to familiar plan, I have to pay all the mensuality or just the difference.

No worries. If you upgrade your subscription from regular Premium to Premium for Family, your payment date will likely be a few days earlier than it had been. This is because of the difference between the price of Premium for Family and the regular Premium subscription you were on.

You won't be double-charged, but instead, the time that you paid for regular Premium will be converted into Premium for Family plan time. You can track all this on your subscription page.
