Premium Family is a discount plan for up to 6 family members who live together. Each user on the plan has their own account, so no one shares a password and everyone keeps their own saved music and playlists. The Premium Family subscription includes Family Mix - a playlist based on the tastes of each member. The plan’s manager can also choose if members get explicit content.

Premium Duo is a subscription plan for two people living together. It includes Duo Mix - a playlist based on the tastes of both people on the plan. Since this plan is designed for only two users, its price is lower than the Premium Family price. 

These subscriptions are similar to each other but differ in price and member capacity. The purpose of the two being available is to offer users a broader variety of plans including different features.

Can I block explicit content on just one of the two accounts of Premium Duo?

Currently there's no Spotify feature allowing you to block the explicit content on just one account in a Premium Duo plan. However, if you think your suggestion will appeal to other users you can always create an idea on the relevant idea board.
