Wife can not join family plan

 My Question or Issue

 My wife can not join the family plan. Appartenly I got an E-mail where I had to confirm her but I somehow deleted it and now when she fills all the information on the form, she gets a red box where it says she can not join the plan.

Please Help!!!!
