I have his email address to setup an account, but where on your website do I click to setup annual account ?

There no annual subscription to Spotify at the moment.

A Premium subscription will renew itself every month till a user decided to cancel it (Terms and conditions).

However, giftcards might be a good idea in case they are available for use in you country.
