Can't accept Duo invitation

Latest iOS and macOS, latest Safari/Chrome

I've sent a Duo invitation to my wife and she can't verify the address on any device or browser. We've tried iPhone 6, iPhone XS and latest Macbook Pro. Neither entering the address manually and using location services doesn't work. We've tried on 4G and Wifi and doesn't work too. What to do now?
