Credit card address verification

My credit card has started being declined every month. Sometimes I can log in and reenter my cc info and the payment will go through and other times I can't. I called my bank and they told me it's being declined for a failed address verification.  I can't find anywhere in my profile or on the payment screen to check or change my address.  This problem started within the last six months and my home address hasn't changed in over three years.  My premium has lapsed and I'm back on free.
