iPad isn't playing my premium subscription

I have premium but when I log in on my iPad it lists my subscription as free. I looked into settings. Tried logging out and then back in. No luck. How do I sync my devices so that it recognizes my subscription?

Is the family plan $14.99 total or this price plus the $9.99 premium fee? That would be a total of about $25. Is that right? Or does the $14.99 monthly fee replace the monthly $9.99 fee?
