Playlist Cover Changing By Itself

So I changed my playlist cover on my laptop, and then on my phone it kept going back to the original cover I had. So I deleted the app from my phone and got it again and it still kept changing the cover and on my computer it changes to. So I don't really know what to do because I have to change it several times and it never stays.
