Spotify setting all of my playlists to not available offline & tracks need to be re-synced

So this has happened a bunch of times - I've got a lot of music I own, and I sync it from my PC to my phone, with all the playlists set to available offline.

Then some amount of time passes (I don't notice how much), and all the files appear to have been deleted from my phone, and none of my playlists are set to available offline.

I am aware of the 3-device maximum; however, I only use one device offline ... is it possible that Spotify is, for some reason, registering the same device as multiple devices?

I have already tried a clean install, I have enough space on my memory card, and I'm not syncing more than 3,333 files.

Any help is appreciated -- I do have a lot of files, and re-syncing is a giant pain.

Thanks
