Family Plan Confirmation

I'm trying to set my wife and me up on a family plan. We both previously had premium subscriptions, so I didn't see the point in paying $23/mo (for some reason she had her sub through Itunes so it was 12.99/mo instead of 9.99). I set up the family account through my account and sent her an invite FROM MY ACCOUNT. The problem is everytime we click on the link to confirm, it says "Congratulations you are about to join the family plan of [wife's name]" instead of my name, who sent the invite. Obviously, it says an error occurred, and won't confirm. I've been dealing with this issue for about a month now and will just get fed up and quit trying. I'm determined to get it fixed today, so any help or insight would be greatly appreciated.

I should also note that we cancelled her premium account through Itunes this morning

Sounds strange. What happens if you try to clear the web browsers cookies and cache? Or try to send the invite from another device, and accepting it from yet another device?
