There are many reasons for that can happen, but maybe it could be because your family members didn't confirm the verification that Spotify sent to their e-mails, so they must do that first. After that I recommend follow this steps to insured:
1. Login from your family members account's on the spotify website
2. Go to https://www.spotify.com/family/verify
3. Confirm postal code and anything else that asks you.
4. Done, confirmed. How they are outside of the family suscription, send the invitation to them again, fill the info, and it should be OK!

I can't add my youngest son to our family premium. I tried doing everything you said and I was successful with my other children but not with my youngest.
