Family premium account verification

Family premium account verification

I have been trying to verify my family premium account for the past couple of days. But it's telling me that the information I am putting in is wrong. It asked me for my name, (first and last) street name, street number, zip code, and city. When I press submit, it just pops up with a red rectangle saying something like "ooops something went wrong try again" has anyone else had this problem? Any tips or ideas would be great?
