This can be done on both Android and iOS devices. At the top of a playlist, tap on the three dots. From the list of options, select "Edit playlist":
Once you've selected this option, you'll see a page where you can edit the name of the playlist and the songs in it. You can also add a description or change the cover of the playlist from the same page.
