Submit button not working in sign up.

Trying to set my son's phone up I am able to get to the page where it says

'Congratulations! You are about to join the family plan of *****. To confirm that all members of the plan are part of a family, please enter your address below.'

I fill out the form but the confirm button at the end of the page doesn't respond. I've tried on different browsers, computers, phones and also tried in incognito but nothing has worked since Christmas day.
