Reviewer did not update app when re-reviewing quota extension

I have submitted 2 quote extension requests now and the first one was denied after a month and there were indeed some changes we needed to make. We submitted again with a new version of the app and another month later we were denied for the exact same reasons. I could see in our TestFlight that the reviewer did not update the app to the newest version, they just reviewed the same version twice.

I am worried if I submit another extension request they will just review the same version again and another month will go by. Has anyone experienced anything like this? I sent an email to the design guidelines and responded directly to the denied extension request email.

What was the reason for the rejection? What is your app idea?

No it was brand/design related things like a wrong corner radius on the album cover art, overlaying things on cover art, etc.
