Yes I am. Same email, same user name and password. I only have one account.
I know this seems redundant, but can I have you log out of both and then log in with your username and password (don't use the email) on both?
Done that. No difference! Web is Premium, App is Trial.

Is the other information (other than the subscription) the same between the web and app? Like, do they have the same playlists?
Yes, both devices have the same playlist. Info are the same, except for the subscription. I have tried logging off and back in, and restarting the phone, and reinstalling the App on my iphone, I still get Premium Trial. I hope my App account will be upgraded to Premium (for family) after 7 days automatically. Initially, I had lots of problem signing up too. Took many attempts before my account got accepted as part of the family package.
