Settings missing in Premium Subscription

My subscription page says "Premium" and I've logged out and back in. Under the top-right username dropdown, there is not "settings" and only "Account" and "Log Out". Where are the settings? Lame.

Audio streaming quality. The settings should be there but they are not. This is a premium account. Why pay for Premium and then stream low quality?
