How do I get premium with iTunes as it's not showing the option on the app

Unfortunately, it's not possible to subscribe through the iTunes Store right now. You can still subscribe through the Spotify website on: http://spotify.com/premium/

Hmm... I don't know when (or if) it'll be back. In the past during Spotify's introduction offer, users would accidentally subscribe through the app instead of the website causing a lot of confusion. This decision was made to make the offer go more smoothly. Sorry for the inconvenience, but it's not possible right now.
