The spotify radio will not load on my iphone 4s running 5.1.1 nor will it give me the chance to search within the Radio tab of the app. I'm running on AT&T 4G
I have tried with no success:
Launch radio from playlist
Launch radio from single song
Close all background apps
Re-install on the iphone
Hard Reset of the iphone
The app will play my playlists and other search music with no problem but the Radio just doesn't want to get passed the loading screen. The radio works fine on my wife's iphone.

I just confirmed - you have to delete both SQLite databases.
What files/dirs do you see in your Spotify app, demeeder? What files/dirs do you see in the Library directory?
I should mention... if you copy the entire Library folder to your workstation, you can use whatever find/locate filename tools and search for anything "radio" with radio in the name. Then go back to your iPhone drive utility and delete those files (again, make sure Spotify is NOT running on your iPhone before you connect it to your workstation!)
It might take awhile to copy - especially if you have a lot of offline music files, but it'll be worth it if you can get the Radio function working!
I appreciate your attention to this. There is nothing with "radio" left in the library folder. I have the library folder mounted and did a search for "radio" with noting showing. Looked in all the locations you mentioned. Ensured Spotify was not running.

(lots of "radio" stuff in here, but think that is all part of the app shell.
okay.. that did not translate my use of the tab button. Bottom line, no "radio" files save the ones that are part of the app shell.
Let's try this next: backup and blow away Library/Preferences/com.spotify.client.plist (there's some radio stuff in there).
And then re-run Spotify. If that doesn't work, kill it again, and put your backup back into place.
If that still doesn't work (I'm taking a stab in the dark now), there's the possibility that one of your playlists might have gotten corrupted and is causing the problem (since the radio tries to add things to your "Liked in Radio" playlist)
I would go through your playlists and remove the offline mode on any of them. Then I would kill spotify, remove all the files Library/Application Support/Users/username (at least all the .bnk files) and try again.
Make sue to back these up before hand. However, it should sync and re-download all your playlists once you start the app up again (and then re-mark for offline use any playlists you had like that before and it'll download your music again).
Any update/luck with my last suggestion?

A new version of Spotify for IOS was released today. I don't see anything specific to the Radio problems mentioned here, but maybe it'll help to try upgrading.
Nope, installed the update. Still doesn't work. On a 4S 5.1.1, I've ried deleting the files, a restore, a reinstall, and an update and the radio still doesn't work.
Have you tried deleting your "Liked from Radio" playlist? (grasping at straws on this one) What program did you use to access the file structure on your IOS device?
I wrote earlier that I suspected of a conflict with some other app, such as Pandora. The theory was discarded because other people are also having the problem even though they do not have Pandora installed.
However, I can't helpt but feel that there seems to be some kind of interference problem with these Internet Music Services. The reason I keep thinking this is because I used to be able to switch flawlessly between at least three frequently-used music services: Radio Paradise, Pandora, and The Bay (plus iTunes, of course). Never had any problems until I installed Spotify and started trying to use the radio.
From that moment on, Pandora started giving me weird problems (I've had to re-install it twice), and of course Spotify Radio stopped working after the first couple of times.
I have not done any consistent research, and I have not tried removing Spotify altogether, always hoping it will get fixed some time soon, so maybe this is just a red herring. But on the othe hand, I've developed a bit of a sense of smell for hidden causes after years of troubleshooting experience, and I can't get rid of that nagging sense that there's a conflict in there somewhere...
Same here. I just did clean install on fresh 5.1.1 new everything. I can't get the radio to stop spinning and playing music :(.

I realized that my data plan saver ONAVO EXTEND caused my Spotify Radio to stop working.
Uninstalling the profile in Settings=>General=>Profiles, then killing the Spotify app and then rebooting the phone made it work.
So if you have a data plan saver, maybe you should try to uninstall it.
Now that I found out what was causing it, it all becomes clear. It seems that ONAVO EXTEND compresses my Spotify data som much, the radio won't work. Or maybe the cause is something else. Anyway, this solution worked for me.
It looks like you're on the right track, and it makes a lot of sense that a network utility could cause this. I went from AT&T to Verizon and the big V doesn't support Onavo (at least when I switched), so I can't install it. But as my phone is jailbroken, I may have installed something else on it that puts itself in the middle of the network stack.
As far as networking utilities, I have NetTalk, OpenSSH, and I had MyWi installed. I immediately uninstalled MyWi in hopes it would fix this, but same issue. I then tried to reset my network settings to no avail.
Can anyone think of anything else that we all may have on our phones that could be causing a conflict?
I actually didn't even know what Onavo was until reading this thread.. As far as I can tell it's not installed unless it's a 5.1.1 specific thing ?
No, it's a program on the App store.
All right, I am positive now that Spotify conflicts with other apps, even more so after reading the posts about conlifct with Onavo.
My Pandora kept breaking down and malfunctioning after I installed Spotify. I tried re-installs and hard restarts to no avail. Finally I removed Spotify from my iPhone. Problem solved: evething else behaves nicely and keeps on working fine.
