 My Question or Issue

If I have a family premium account and can add 5 other family members, can someone with an existing account  use one so they don't lose their settings or Playlists

Yes, that's correct! They can use an existing account to join Premium Family, keeping their saved music and playlists. Other members of the plan also use their own accounts, so they won't need to share login details.
