My Question or Issue
My Spotify adds songs to and removes songs from my Playlist on its own. I'll go to find a song I want to play and it's gone when I knew I had it added. Or I'll be on Playlist and an unfamiliar song plays, I check and it's added to my Playlist when I know I had never added it. Very tired of these things happening. My Spotify also happens to add songs in on "smart shuffle recommendations" when I clearly have smart shuffle turned off for the Playlist I am in. And yes, of course it goes ahead and usually adds that song to my Playlist.

Have you tried any troubleshooting steps so far such as logging out > logging back in twice in a row for example? This step can help resync your device and account so you can start here.
On another note, did you notice when this started happening? Was it after an update of the app for example and does the issue persist on another device? Does it happen with multiple playlists or with a specific one?

Just to make sure I understand you correctly, does the Smart Shuffle button turn itself on after some time even if you've disabled it or does the button stay disabled, but you're still seeing songs being added to your playlist via Smart Shuffle?
It'd be great if you could also send a screenshot or two of how things look like from your end so we can take a look. You can attach them to your next response by using the Insert image option in the post editor (just make sure not to send any private info).

To continue investigating, could you let us know if this happens across your devices or only on the mobile phone? If this happens only on a specific device, could you send us a screenshot where we can see the playlist with the songs that weren't added by you? Also, does this happen only with a specific playlist or multiple ones?
An additional step you can try is to disable Autoplay to see if that'll change the behavior of the app. If it's enabled, you'd hear recommended songs when you have songs in the play queue.

Could you give it another try and instead send a short screen recording which captures the described behavior in action? That way we can get a clear idea of what's happening on your end.
If you're unable to attach it here in your reply, you can also upload it to a cloud like Google Drive and share the URL - just make sure the file is accessible by anyone with the link.
In the meantime, it's worth checking if there are any apps connected to your Spotify account and removing them temporarily to see if that does the trick. If you suspect that your account has been compromised, I strongly recommend securing it by following the steps here.
