Using Desktop version on Windows.
Some months ago the artwork stoppeed working in the main browser window. No Artist or Album thumbnails show. Artist Image gallery no longer displays photos.
However, in the smaller left bottom window showing current album playing it is display.
There have been numerious updates but none have fixed the issue.
I've uninstalled and reinstalled several times but none have fixed issue either.

That's a peculiar issue!
I'd suggest looking into the hosts file first:
1. open Notepad as an administrator (right-click - Open as Administrator)
2. go to File - Open and navigate to
C:\Windows\System32\drivers\etc
3. Choose "All Files" instead of "Text Documents" next to the File Name box. You'll see a handful of files. Pick hosts and click on Open.
4. The file is opened. You will see a block of text with # in front of each line and might see entries like
0.0.0.0 website.com 127.0.0.1 website2.com
Check for any entries with spotify or fastly in the address. Examples may look like:
0.0.0.0 weblb-wg.gslb.spotify.com 0.0.0.0 prod.b.ssl.us-eu.fastlylb.net
(and any variations inbetween)
5. If you do find any, you can delete the line. You can also add a # in front of the line, that's 'commenting out'.
6. Save your changes. Restart Spotify.
You can also try removing all lines to see what happens.
If that didn't help - does it happen in your mobile devices or the web player as well? 🙂

On the Windows PC followed your example using Notepad view all files down to etc folder. There are no files in the folder.
Checked web player on same windows PC and artwork is fine.
Checked iOS play on iPhone and artwork is fine.
Next thoughts ?
Did you make sure to look for "All Files", not "Text Documents"?

I'd suggest trying out a clean reinstall, maybe the app itself is faulty:
1. Close Spotify and uninstall it.
2. Go to %AppData% in Windows Explorer, and delete any Spotify folders you find in Local and Roaming folders.
3. Restart your computer.
4. Install Spotify.
Note: if you run a Windows Store installation (on Windows 10), chances are there is nothing in Appdata. Try resetting the app's data as described here (the Windows Club), and reinstall following the steps in this article.
