Same problem here (Windows 10) - it doesn't happen on every song change, but I particularly notice it if I haven't played anything for a bit (an hour or so) then come back and press play - it plays at full blast, even if the volume is muted.
Given that some may be used to playing Spotify at a lower volume and have their PC sound set up for that, there's scope for hearing damage here. This is a massive bug in the latest version - you need to fix this, and fast.
I have to stop using spotify now because of the ringing in my ears this bug has caused me. Thanks, spotify!
The only current fix to this bug right now is to revert back to an older version. It works until the second or third time you open the app because then it auto updates...
This is why there should be an option in settings to turn off auto update, in case some users experience issues with the newest update.
it's annoying that there was an update yesterday but the update didn't fix the bug!

Try reinstalling. For me (Win10 with USB DAC) the new version 1.2.47.366 in the Windows Store seems to have fixed the issue. Will report back if it turns out it didn't.
Check your version via the three dots button in top left > Help > About Spotify.
yes! I just closed it completely and reopened and I think it's fixed!!!
the only bad change still is that when you switched to video before, it would continue to play videos for the next songs (when available) but this version has stopped doing that (why Spotify - there should be an option on the Desktop to "always play video when available")
where do you founded the older version? and which version are u using?
Mine is fixed now too, I installed the desktop app again (without Microsoft store) and have no more problems!
I installed the below version:

from Microsoft Store and it did fix the max volume issue.
The direct download from Spotify website however is still showing the problematic version 1.2.47.364.gf06e5cee for me. Best to avoid that one until Spotify pushed ver 1.2.47.366 to its direct download link.

I've downgraded back to 1.2.47.366.g0d3bd570 and no more volume issue.

It is tied to the same issue everyone else here is facing and is hoping gets fixed within next patch.
Turning PC on, switching between listening devices and leaving the PC idle(for some weird reason, when screen goes into power saving mode) trigger this bug.
None of the reccomended fixes help this, since the volume can get crazy high sometimes, I would suggest using the web version or make sure to mute/pause and adjust the volume before it blasts your ears.
