No notifications while listening to Spotify using iPhone and Apple Watch

My Question or Issue

I recently got the Apple Watch 3.  When I run I listen to SpSpotifystreaming from my phone, not using the watch on cellular mode, also using BlBluetootheadphones) and I have tried various fitness apps for tracking my runs (Map My Run, Nike Running Club, as well as Apple Fitness).  None of these apps will send audible notifications  to my headphones.  Typically I get updates every mile telling me my overall time, overall mileage, split times, etc.

iphiphoneider is up (system is not on mute) volume is up, music is clear and loud.  Just no notifications.

I have tried everything I can think of and I am out of ideas. Thanks!

I should have also noted that I have all notifications turned on in the watch and phone.  Works fine with just phone.
