I have Premium. Do I have to pay to download the app on my Ipad?

The Spotify app is free in the app store for any supported Apple device. You should never be required to pay to download it.

Everytime I try to download the app, it brings me to a page for my credit card.

Hmmm, that shouldn't be the case if you are in the authorized Apple store for your device.
If so, have you checked your payment info that's on file for Apple? It might be something within the app store account settings and not asking you to pay for the app itself.
The Spotify app is free to download direct from Apple and you should be able to log in with your current Premium credentials with no additional charges.

I click on "get" the app. Then I click on "install" and the next screen that comes up, has my name and credit card info and home address. Now, let me say this, the credit card info showing, is out of date. I'm afraid to update it because I don't want anything being charged to me. But you think if I update my info, that it will let me download the app and not charge me?

Ok, this sounds like Apple is just wanting you to update/confirm your account data with them. They have done this to me before when I had no payment info on file or back when I had previously subscribed to Apple music.
If you update your details with them this will only be on file for apps and services that may require a purchase, but they will always make you confirm before truly authorizing an app purchase (unless you have a purchase exception set up for this.)
Once you update your account data, you should be able to download the app at no charge. Spotify is always a free to use app whether or not you have Premium.
