1 month free trial premium but why do I have $3.99 balance and my card got declined. I cant remove the card to itunes and I cannot download/update an app unless I settle that balance. Please help!!!

Did you try to get the Premium trial through iTunes? If so, then because the payment is processed through Apple and not Spotify it is priced differently and is exempt from that promotion.
